FERPA - Student Authorization Form

The Family Educational Rights and Privacy Act (FERPA) grant students certain rights concerning his/her education records.

Notwithstanding these rights, the student may release his/her educational records to parent(s) or guardian(s) for the purpose of keeping them informed about the student's education at ³ûÄñÊÓƵ.

Education records include, but are not limited to, information about student academic standing, disciplinary issues and financial obligations to ³ûÄñÊÓƵ.

This consent will remain in effect until the student graduates or withdraws from the ³ûÄñÊÓƵ.

The student may submit a subsequent notification in writing directing Kentucky State University to no longer release information to any or all of the authorized individuals.
